METAIODOBENZYLGUANIDINE (MIBG) SCAN OF WIDELY METASTASIZED NEUROBLASTOMA Figure 11-5 METAIODOBENZYLGUANIDINE (MIBG) SCAN OF WIDELY METASTASIZED NEUROBLASTOMA A 2-year old girl presented with fever and a painless swelling in the abdomen. Blood and urine cultures were negative and initial ultrasound was suspicious of a renal tumor. After referral to an expert center, clinical examination (slight hypertension), laboratory testing (increased urinary catecholamines), and abdominal MRI imaging showed that a diagnosis of metastatic NB was more likely. The MRI depicted a right-sided adrenal mass with retroperitoneal lymph node metastases. Additional MIBG scintigraphy shows increased uptake in the primary adrenal NB and locoregional lymph nodes (black arrow), but also extensive bone marrow metastases throughout. At the top, is a classic orbital metastasis and at the bottom, a tibial metastasis (red arrows).
